Is a Panoramic X-Ray Actually Necessary? And What Should It Cost?

A panoramic X-ray (sometimes called a "pano") is the large full-mouth image taken on a rotating machine that swings around your head. It shows all your teeth, your jawbones, sinuses, and surrounding structures in a single image.

When it's clinically appropriate: A panoramic X-ray is indicated for new patients, for evaluating wisdom teeth, assessing jaw conditions, evaluating bone levels in periodontal disease, or planning implants or orthodontic treatment. Most practices recommend one every 3–5 years for established patients.

When it may not be necessary: If you had a pano taken recently at another practice, your new dentist can request those records rather than retaking the image. Always ask when your last panoramic X-ray was taken and whether your records can be transferred before agreeing to a new one.

New patient exam bundling: Many practices include the panoramic X-ray in a new patient comprehensive exam fee. Others bill it separately. If you're a new patient, ask upfront whether X-rays are included in your new patient exam fee or billed additionally.

What to Watch For

A recommendation for a panoramic X-ray at every visit, or more frequently than every 3 years without a specific clinical reason, warrants a question. Routine annual panos are not standard of care for most adult patients with no active problems.
