The Number That Matters Is the Total, Not the Per-Quadrant Price
Deep cleaning — technically called scaling and root planing (SRP) — is the most commonly recommended periodontal procedure, and also one of the most commonly misunderstood in terms of total cost.
D4341 is priced per quadrant — one quarter of the mouth. The mouth has four quadrants. Most patients who need SRP need two to four quadrants treated. A per-quadrant price of $220 sounds manageable; four quadrants at $220 is $880. Always ask how many quadrants are being recommended and get the total figure before you agree to treatment.
Why some patients get multiple quadrant recommendations: SRP is appropriate when there is documented bone loss and pocket depths of 4mm or greater around teeth. If your dentist recommends four quadrants of SRP, ask to see your probing chart — the measurements taken by the hygienist — and ask which specific teeth exceed the 4mm threshold. A legitimate recommendation is backed by measurable clinical data.
Anesthesia is sometimes included, sometimes not. SRP is typically done with local anesthesia. Some practices include this in the quoted price; others bill it separately. Confirm before the procedure.
Periodontal maintenance visits (D4910) typically follow SRP — usually every 3–4 months instead of the standard 6-month cleaning schedule. These cost more than a regular cleaning and are a recurring cost. Ask about the expected maintenance protocol before beginning treatment.
What to Watch For
SRP is a legitimate and necessary procedure for patients with actual gum disease — but it is also one of the more frequently over-recommended procedures in dentistry. If your gums don't bleed, you have no bone loss on X-rays, and your pocket depths are mostly 3mm or less, a recommendation for four quadrants of SRP warrants a second opinion.
